Request Assistance - Parents & Carers

School of Special Educational Needs: Sensory

Parents and carers can refer a child with diagnosed hearing loss, vision impairment or dual sensory loss.

SSEN: S is guided by the World Health Organisation definitions of hearing loss and vision impairment, additionally holding a holistic view of child development and the functional impact of the disability.

Please refer to our eligibility guidelines and request assistance (below) as soon as you receive a diagnosis. The following information is required to determine eligibility for services:

  • an audiogram and accompanying report (for hearing loss).
  • an ophthalmologist report or optometrist report (for vision impairment).
  • an audiogram and accompanying report, and an ophthalmologist or optometrist report (for dual sensory loss).

Following your Request for Assistance, SSEN: S will email a reply including a referral form and further details. To progress the referral, the completed referral form and relevant health report/s should then be provided.
